Abstract. Statistics shows lack of satisfaction, by citizens to quality of medical services, this may be due to today’s medicine is focused on human as material object and at the expense of subjective. So from one side person and his psyche is becoming the reason for development of the disease it may be psychogenic and psychosomatic, from other side not assessable assistant of doctor and patient in getting rid of the disease, with manifestation of interest and activity. In article has been given view (result statistic) which was provided by medical and institutional researchers about the reality of the situation, as well as practical examples of psychotherapy. Although problem is not using the potential of psychotherapy: but fears, suspicions etc hinder the staff of internal affairs on time and properly rehabilitate their health and psyche, eliminating emotional and vegetative strain.

Key words: psychotherapy, psyche, prevention, professional burnout syndrome.

PDF downloadControl of systemic inflammation cellular component by T-regulatory cells in time of exacerbation patients with mild and moderate stage of chronic obstructive pulmonary disease.

O.A. Enikeyev, V.I. Nikulicheva, Sh.Z. Zagidullin, S.A. Enikeyeva

Abstract. Estimate infuence of Tregulatory cells on systemic infammation cellular component in dynamics of exacerbation in patients with mild and moderate stage of chronic obstructive pulmonary disease (COPD). 21 patients participated in research with mild and 21 patient with moderate severity level of COPD. Control group consist from 20 conditionally healthy faces. Signifcant decreasing of Tregulatory cells (CD+4, CD+2 +5) and increasing of T-NK cells (CD+3, CD+1 6, CD+5 6) supervised in both groups in all times of observation. In all patients found leucocytosis and signifcant rise of B-lymphocytes after 10 days of therapy. Conventional therapy of COPD exacerbations in hospital did not affect on restoration of balance between proinfammatory/infammatory cell subpopulations in patients with mild and moderate stage of this disease.

Key words: Т-regulatory cells, chronic obstructive pulmonary disease, systemic infammation, immunology.

I.V. Leshchenko, A.V. Krivonogov

Abstract. Studied the characteristics of the course and complications of community-acquired pneumonia (CAP) for pandemic infuenza A/H1N1/09 based on clinical and laboratory data and radiology. The study involved 250 patients in 4 groups: Group 1 — 57 patients with uncomplicated VP, Group 2 — 124 patients with CAP, and the systemic infammatory response syndrome (SIRS), third group — 53 patients with CAP complicated by severe sepsis, and 4 group — 16 patients with CAP with septic shock. All patients received antiviral, antibacterial and symptomatic therapy, oxygen therapy was carried out by the testimony and artifcial lung ventilation (ALV). In 225 patients (90,0%) have 2-way loss of lung tissue. 193 (77,2%) patients required oxygen therapy, 108 of them (56%) were hospitalized in the intensive care unit and intensive care unit (ICU) (15,6%) carried out artifcial ventilation (mechanical ventilation). Overall mortality was 10,4%, among obese patients — 30,0%.

Key words: community-acquired pneumonia, pandemic infuenza А/Н1N1/09, clinic, complication, treatment.

PDF downloadLung and airways electrical impedance measurement on different probing current frequences or lung functional testing study

V.Ju. Mishlanov

Abstract. This paper is devoted to clinical results of new lung function diagnostic method estimation. It’s based on electrical impedance of airways and lung parenchyma measurement on different probing electrical current frequencies in the presence of 0,9% sodium chloride solution nebulized inhalation. The theoretical foundation of new diagnostic method is the result showing electrical impedance increasing after local obstruction of the main probing current conductor. Program-apparatus complex «BIA-lab Spiro» was used in present study. Material of study: 10 healthy and 10 asthmatics were examined by classical spirometry technique and new polyfrequent electrical impedance spirometry method. In the group of asthmatics signifcant increasing of electrical impedance module value on 5 000, 10 000 and 20 000 Hz probing electrical current frequencies was revealed. Signifcant correlation links were shown between FEV1, some airfow speed markers and airways electrical impedance module value.

Key words: electrical impedance, spirometry, bronchial asthma.

Abstract. Clinical case of the male patient 44 years old is presented. In this patient one day after elective coronary stenting acute thrombosis of the stent has developed. Thrombosis was successfully terminated by intracoronary infusion of 30 mg alteplase. Clinical studies of modern using intracoronary thrombolysis (ICT) with concomitant are reviewed. Available data allow us to believe wider usage of ICT in the near future.

Key words: intracoronary thrombolysis, percutaneous coronary interventions, coronary stenting.

PDF downloadLong-acting β2-adrenoceptor agonists — clinical profile and safety

A.A. Vizel, I.Y. Vizel, R.I. Shaymuratov

Abstract. The safety and clinical profle of inhaled long-acting β2-agonists: a systematic review. Possible mechanisms of long-acting β2-agonists are also described. In addition to existing long-acting β2-agonists (salmeterol and formoterol) we present data about novel β2-agonist — indacaterol with a 24-h duration of action.

Key words: long-acting β2-agonists, safety, salmeterol, formoterol, COPD, bronchial asthma.
